NURSE CASE MANAGER

POSITION SUMMARY: The Nurse Care Manager will work to improve the overall health of individuals with Serious Mentally Illness (SMI) throughout Monmouth County. Work in partnership with other service providers to coordinate services for participants. Develop relationships with referral sources and partner organizations.
ESSENTIAL FUNCTIONS:
- Travel throughout the county
- Effectively communicate and present BHH services to small and large groups
- Demonstrate clinical competence in the nursing process
POSITION DUTIES AND RESPONSIBILITIES:
- Coordinates services in consultation with various medical teams and support staff to ensure the provision of high quality and efficient care to consumers and families
- Represents and markets Easterseals New Jersey Behavioral Health Home Program in the Mental Health Community in Monmouth County
- Conduct thorough and comprehensive health assessments
- Focuses on health promotion and disease prevention; provide care coordination of behavioral and medical health
- Identifies risk factors and needs, utilizing collaborative care models to improve patient outcomes
- Monitors effectiveness of interventions
- Accurately and appropriately documents prescription refill information including name of medication, dosage, route of medication, quantity, and dispensing instructions
- Engages in a clinical practice that accurately reflects NJ Board of Nursing regulations and Code of Ethics
- Participates in required QA programs/initiatives as directed
- Assists in data collection and analysis
QUALIFICATIONS, SKILLS, AND EXPERIENCE:
- Must hold a current RN NJ License
- At least 3 years of clinical experience in behavioral health field working with individuals with severe and persistent mental illness
- Possess excellent communication skills
- Knowledge of Electronic Health Records helpful
