What are the responsibilities and job description for the LPN - Home Health and Hospice (FT - Days) position at Excelsior Springs City Hospital?
The LPN is responsible for the application of principles of the nursing process for a specific group of patients/clients, for directing the care given by team members in home health and in-home services, and certain departmental committee work and operation activities. On occasion, the LPN may be assigned to a special responsibility in community awareness activities. The LPN demonstrates a positive attitude toward the responsibilities of a specific role on the health care team; and contributes clinical competence and a caring attitude in supporting a healing, professional practice environment.
Responsibilities:
- Provides nursing care for patients/clients utilizing the nursing process.
- Assesses, administers, supervises, documents, and evaluates the nursing care needed and given to the patient/clients, utilizing the care plan.
- Completes the chart within specified time frame.
- Sets up patient’s folder in home.
- Initiates discharge planning on admission and reevaluates as condition warrants.
- Documents the following: observations; demonstrations, exceptions, valid complaints and team member communications.
- Observes and reassesses as ordered coordinated lative to diagnosis, needs, and changes in condition to keep physician informed and accurately coordinate care.
- Demonstrates knowledge of the principles of growth and development over the life span; possesses the ability to assess data reflective of the patient/client’s status; interprets the appropriate information needed to identify each patient/client’s requirements relative to his or her age-specific needs; and provides the care needed as described in the department’s policies and procedures and as ordered by the physician.
- Properly administers medications; knows side effects, signs of toxicity, and what vital signs to check or lab work needed to monitor.
- Performs IV therapy per protocol.
- Provides nursing care for patients/clients utilizing the nursing process.
- Assesses, administers, supervises, documents, and evaluates the nursing care needed and given to the patient/clients, utilizing the care plan.
- Significant changes in condition are reported to the physician, the supervisor, other team members and the responsible party in a timely manner and documented.
- Actively participates in case/team conferences.
- Provides patients/clients/family with accurate, appropriate, and unbiased information concerning other services and referral agencies.
- Assists patient/client/family in identification of risk factors with accurate information to identify potential illness or injury and what to do in these events.
- Oversees the maintenance, stores, and charges for equipment and supplies, both in the workplace and in the patient/client’s home.
- Demonstrates isolation techniques (per CDC) and disposal of hazardous wastes in the home.
- Performs other duties and responsibilities as assigned.
Position Requirements:
- Current LPN licensure in the State of Missouri.
- Maintain Basic Life Support (BLS) issued by the American Heart Association or preapproved provider.
- 2 years of relevant experience preferred.
