Family Tree Job Description
Position Purpose: To increase the stability of families receiving Child-Only TANF by providing short-term case management, crisis intervention, referrals to community resources, financial assistance, and emotional support and improving their knowledge and accessibility to community resources available in their community.
Essential Duties/Responsibilities:
- Initiate contact (e.g., cold calls) with potential clients based on the county's referral list.
- Conduct home/phone/virtual client interviews to complete need-based assessments and determine how the Kinship Program can help maintain family stability and increase their knowledge of community resources.
- Based on funding source requirements, maintain a full caseload of clients/families; this may require seven to eight monthly intakes.
- Collaborate with clients in developing individualized plans at program entry/exit and assist their families in meeting their goals.
- Respond promptly to clients' calls about crisis/problem situations and provide appropriate resources and financial assistance to families for their identified needs based on completed or requested assessments.
- Follow Kinship Program internal policies to create and maintain case notes, complete assessment and paperwork, data entry, and completion of electronic files on all participants.
- Promptly document all services using the Colorado Benefits Management System (CBMS) and Family Tree database, following agency confidentiality policies and security procedures.
- Participate in facilitating and supporting monthly Kinship Legal Clinic and Circle of Parents support groups.
- Participate in outreach efforts (e.g., giving presentations, attending conferences, meeting with ground-level staff, etc.) using marketing material provided by Family Tree to maintain a strong referral base and build a positive reputation and knowledge of the program and agency within the community and with County Human Services staff.
Other Duties:
- Support Family Tree's mission and model Family Tree values of commitment, integrity, accountability, diversity/inclusiveness, collaboration, and advocacy.
- Adhere to all Family Tree established policies and procedures, including timely reporting of suspected abuse or neglect and confidentiality of client information.
- Participate in Team and Agency meetings and training to enhance client experience and services.
- Engage in Individual Supervision to develop and achieve professional goals.

Working Environment:
- This is a blended/hybrid remote/virtual and in-office position.
- Must have access to reliable daily transportation for various off-site client meetings.
- This position schedule requires flexibility and may include afternoon, evening, and, occasionally, weekend hours. This is a 40-hour-a-week position, primarily Monday through Friday.
Physical Activities:
- Sitting/standing for long periods
- Talking, hearing (ordinary conversation)
- Seeing (reading the paperwork, computer screen work, etc.)
- Manual dexterity is sufficient to operate computers and telephones.
- Visiting homes/apartments that may or may not be handicapped accessible.
