What are the responsibilities and job description for the Community Health Worker position at FIRST 5 Mendocino?
Position Summary
The First 5 Mendocino CHW provides support services directly to families. The First 5 CHW will have lived experience in pregnancy, birth, parenting and/or fostering. They will support families from pregnancy through early childhood to improve whole person/whole family health and well-being engage through:
- Health education, navigation, appropriate screening and assessment.
- Referral and connection to appropriate services covered under the Partnership Health Plan benefit
- Empowering parents as advocates
- Assist in the development of plans to access health promoting resources using problem solving methodologies including effective communication techniques.
- They will network with local service agencies, prepare monthly and annual statistical reports and other duties as assigned.

- Knowledge and support for early childhood development.

Minimum Qualifications:
- Lived experience navigating pregnancy, early childhood, and parent supports with understanding of a wide range of challenges and barriers to support and resources as a parent or caregiver.
- Ability to complete a First 5 Mendocino approved CHW Certificate program. * Knowledge or willingness to learn about early childhood development. * Ability to travel throughout service area as needed.
- Consistent and professional work demeanor.
- Works effectively under pressure and can handle multiple assignments and interruptions.
Education and/or Experience
- High School diploma or GED
- A combination of education and experience that would likely provide the qualifying knowledge and abilities.
Language Skills
- Ability to read, write, and speak English.
- Ability to read and interpret documents.
- Ability to document interactions and plans.
- Bilingual in Spanish desired.
Mathematical Skills
- Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
Reasoning Ability
- Ability to apply common sense understanding to carry out complex instructions furnished in written, oral, or diagram form.
- Ability to navigate a wide range of challenges and barriers to support and resources.
Computer Skills
- PC computer competency, including experience with word processing using MS Word, Power Point, and the internet.
- Ability to use a smart phone and other technologies.
Certificates, Licenses, Registrations
- Pass the fingerprint/Dept. of Justice background requirements.
- Possess or obtain upon employment, a valid California Driver License with a clean driving record.
- Provide and maintain proof of automobile insurance.

FIRST 5 Mendocino will offer a competitive compensation package including base salary, comprehensive health benefits package, retirement plan, and vacation benefits
Job Types: Full-time, Part-time
Pay: $27.81 - $33.80 per hour
Expected hours: 20.01 – 27.99 per week
