Job Summary:
CHW services are preventive health services that may assist with a variety of concerns
impacting patients, including but not limited to, the control and prevention of chronic
conditions or infectious diseases, behavioral health conditions, and the need for preventive services. Additionally, CHW services are designed to prevent disease, disability, health conditions, or their progression; prolong life; and promote physical and mental health.
- CHW services can be provided as individual or group sessions. The services can also be provided virtually or in-person with locations in any setting including, but not limited to, outpatient clinics, hospitals, homes, or community settings. There are no service location limits.
- Eligibility criteria for CHW services are based on the presence of a diagnosis of one or more chronic health (including behavioral health) conditions or a suspected mental disorder or substance use disorder that has not yet been diagnosed.
Services provided:
- Health Education: Promote the member's health and address barriers to physical and mental health care by coaching and goal setting to improve health or the ability to manage one's health.
- Health Navigation: Providing information, training, referrals, or support to members.
- Screening and Assessment: Does not require a license and assists in connecting members to the appropriate services to improve health.
- Individual Support and Advocacy: Prevent the start or worsening of health conditions or injury.
- Violence Prevention: Using a combination of the above-mentioned services to reduce harm.
Responsibilities:
- Conduct outreach to patients in the hospital, outpatient clinics, homes, tele-health, and community settings to provide health education and support.
- Assist patients in navigating the healthcare system and accessing resources for follow-up care and PCP visits.
- Collaborate with healthcare providers to develop care plans that address the patient’s needs to improve health post-discharge.
- Travel to hospitals, patient’s homes, and community locations to assist patients with short- term goals.
- Conduct follow-up calls to ensure continuity of care post-discharge.
- Document patient interactions and outcomes accurately in the electronic health record.
- Participate in multidisciplinary team meetings to discuss patient cases and coordinate care as needed.
- Maintain patient confidentiality and adhere to HIPAA guidelines.
Required CHW Minimum Qualifications:
- CHWs must have lived experience that aligns with and provides a connection between the CHW, and the Member or population being served. This may include but is not limited to, experience related to incarceration, military service, pregnancy and birth, disability, foster system placement, homelessness, mental health conditions or substance use disorders, or being a survivor of domestic or intimate partner violence or abuse and exploitation.
- Lived experience may also include shared race, ethnicity, sexual orientation, gender identity, language, or cultural background with one or more linguistic, cultural, or other in the community for which the CHW is providing services.
- Ability to work independently and as part of a team in a fast-paced environment.
- Empathy, compassion, and a non-judgmental attitude towards patients facing health challenges. Proficiency in basic computer skills and electronic health record systems.
Preferred Qualifications:
- A bachelor's degree in psychology, social work, human services field, or a related field or 2 to3 years of related experience.
- Certification as a Certified Psychiatric Rehabilitation Practitioner (CPRP).
- Applicants who have degrees or certificates in clinical areas other than the mental health field are required to complete an additional 45 hours of training in psychiatric rehabilitation.
- Experience working in a healthcare setting, particularly in an ED or inpatient unit.
- Experience in Enhanced Care Management.
- Bilingual (Spanish).
