Key Responsibilities
- Conduct thorough assessments of patient conditions and the situations of families/caregivers during each visit, covering physiological, psychosocial, and spiritual aspects.
- Identify and address educational needs for patients and their families to enhance understanding and care delivery.
- Administer patient care in accordance with physician directives and agency policies, ensuring compliance with professional standards.
- Complete necessary patient and agency documentation accurately, thoroughly, and promptly, reflecting a clear and comprehensive picture of the care provided.
- Transcribe, implement, and evaluate new physician orders with precision.
- Communicate any patient issues or concerns to the Care Manager/Patient Care Coordinator and other relevant healthcare professionals in an effective and timely manner.
- Perform and document admissions accurately and efficiently.
- Strong assessment and evaluation capabilities.
- Excellent communication skills to effectively engage with patients and team members.
- Ability to educate patients and families comprehensively.
- Proficiency in accurate documentation practices.
- Knowledge of home health and hospice care standards.
- Degree from an Accredited School of Nursing (BSN preferred).
- Current Wisconsin RN license.
- 1-2 years of experience in Med/Surg or Home Care settings.
- Valid Wisconsin Driver’s License, along with a reliable and insured vehicle.
