RN Weekend Home Care (Baylor Nurse)

Job Overview

Horizon Home Care and Hospice is actively seeking dedicated and experienced Registered Nurses to join our esteemed Home Care team as Weekend Visit Nurses (Baylor RN). In this pivotal role, you will be responsible for providing exceptional patient care and ensuring the seamless coordination of services that uphold the values and standards of our home health and nursing practices.

Key Responsibilities

• Conduct thorough assessments of patient conditions and the situations of families/caregivers during each visit, covering physiological, psychosocial, and spiritual aspects.
• Identify and address educational needs for patients and their families to enhance understanding and care delivery.
• Administer patient care in accordance with physician directives and agency policies, ensuring compliance with professional standards.
• Complete necessary patient and agency documentation accurately, thoroughly, and promptly, reflecting a clear and comprehensive picture of the care provided.
• Transcribe, implement, and evaluate new physician orders with precision.
• Communicate any patient issues or concerns to the Care Manager/Patient Care Coordinator and other relevant healthcare professionals in an effective and timely manner.
• Perform and document admissions accurately and efficiently.

Qualifications

• Degree from an Accredited School of Nursing (BSN preferred).
• Current Wisconsin RN license.
• 1-2 years of experience in Med/Surg or Home Care settings.
• Valid Wisconsin Driver’s License, along with a reliable and insured vehicle.
