We are a home health and hospice agency providing services in clients' homes from Green Bay to Kenosha and from Oconomowoc to Lake Michigan. Our home office is located in Germantown, with branch offices in Fond du Lac, Waukesha, Appleton, and Sheboygan.
Our clients reside in assisted living facilities, their private home, or wherever home may be. We bring the care to them.
We are currently in need of a Weekend Phone Triage RN or LPN for our Home Health and Hospice Agency.
Part-time opportunity – 7 AM to 7 PM every Saturday and every Sunday

Job responsibilities include, but are not limited to, the following:
- Telephone nurse triage services
- Respond to health-related questions from patients via telephone by assessing their needs and guiding them through self-management
- Determine whether a nurse visit is required
- Document all communications in the patients’ electronic medical records – we utilize HomeCare HomeBase.
- Excellent communication skills with clients, families, doctors, facility staff, and co-workers is a must to be successful in this role
- Ability to work independently
- Good organization and time management skills are also critical
- Love working with the geriatric population
RNs or LPNs with home health and/or hospice experience are preferred; however, applicants new to the home health and hospice field are welcome to apply. All staff receives thorough training to set them up for success.
