Summary
The Referral coordinator assist patients by coordinating services and enrolling patients into the Genuine Health at Home Program. The Referral Coordinator also performs general administrative or clerical tasks such as answering phone calls and emails, maintaining patient records, and verifying insurance information upon receipt of referral. The Referral coordinator frequently interact with patients, doctors, and insurance companies, and must have the ability to maintain patient records and confidentiality, manage multiple schedules, and ensure that patient information is complete and up to date.
Essential Duties and Responsibilities
- Perform daily outbound enrollment/recruitment calls to patients for Genuine Health at Home program
- Must be able to follow English/Spanish call enrollment scripts
- Manage, track, and coordinate all inbound Chronic Care and Transition Care Referrals
- Route Incoming documents to appropriate department
- Ensure complete and accurate registration, including patient demographic and current insurance information
- Assemble information concerning patient's clinical background and referral needs.
- Scheduling and confirming RN Health Coach calendar for Initial home assessments
- Maintain active communication with participating providers to ensure all patient services and needs are coordinated
- Ensure all referrals are addressed in a timely manner
- Administrative tasks such as answering the phone, emails, faxing, maintaining patients’ records
- Maintain ongoing tracking and appropriate documentation on all referrals
- Maintaining and updating patients’ demographics and current insurance information in systems
- Assists in administrative coordination of care for patients
- Participates in team meetings
- Builds rapport with referring participating providers
- Maintains log of daily outbound/inbound communications with patients and providers
- Adheres to all policies and procedures including but not limited to the HIPAA Privacy rule
- Performs other duties as assigned
Knowledge, Skills and Abilities
- Must demonstrate confidence in overall phone skills
- Excellent Customer service and sales skills preferred
- Must be able to read and communicate fluently in English and Spanish
- Knowledge of Care Coordination for the elderly
- Ability to work at a fast pace with accuracy and attention to detail
- Compassion and empathy for patients, caregivers and families
- Respect for peers, providers of care, and other care team members
- Strong communication and interpersonal skills; both written and oral
- Computer literacy
- Proficiency in Excel and Word
- Proficiency with electronic medical records
Minimum Education and Experience
- Education
- High School Diploma
- Experience
- Two (2) years of experience in an outpatient, direct patient care clinic; or
- Two (2) years of experience as a care coordinator in an inpatient setting
- Language
- Fully bilingual required (English/Spanish)
