JOB DESCRIPTION
THIS POSITION IS BASED IN OUR FAIRFIELD, CA OFFICE: The Housing Case Manager will work to end homelessness for vulnerable populations in our community by providing housing navigation, tenancy and sustainability services to eligible program participants that are homeless or at risk of homelessness and assisting individuals and families in securing and maintaining permanent housing.

Housing Navigation Program Director
Essential Duties:
- Use a strengths-based, trauma-informed care and collaborative approach with residents and other program participants.
- Establishes and maintains effective communication with program participants, building and strengthening relationships. Fosters collaborative relationships with external providers, facilities, community resources, etc.
- Conducts telephonic and in-person outreach and engagement activities to housing insecure program participants identified as eligible for services.
- Completes housing navigation intakes and assessments to identify housing needs and barriers. Identifies and facilitates access to suitable housing options for program participants.
- Supports program participants as they go through housing application processes, move-in activities, rental negotiations and service referrals.
- Conducts facility and habitability inspections, as needed.
- Facilitates access to supportive services such as job training, life skills development, legal services
- financial resources and health care providers, in collaboration with Care Managers.
- Provide crisis intervention, as needed.
- Fosters working relationships with property management firms, property owners, housing
- services agencies and housing advocacy organizations. Engage with landlords and familiarize them with Harmony Homes for Healing’s housing navigation, tenancy and sustainability services designed to support our program participants.
- Effectively manages caseload, complying with policies and procedures, workflows, documentation requirements, and timelines.
- Document all interventions in Harmony Homes for Healing’s electronic record system(s).
- Completes administrative responsibilities: read/respond to emails and tasks.
- Attends staff development sessions, workshops, and conferences, as requested.
- Performs other duties as required or assigned.
Job Requirements:
- Healthy boundaries are a MUST.
- High school diploma/GED required, with preference given to candidates pursuing or holding a BS
- in a related field.
- Residential experience and/or direct experience in providing services to persons/families
- experiencing homelessness, trauma, and/or substance use preferred.
- Personal qualities and skills: good interpersonal and communication skills orally and in writing,
- initiative, positive attitude, non-judgmental, empathetic, compassionate, accountable, resourceful, good time management and organizational ability, good judgment, attention to detail, ability to manage multiple tasks and crises and prioritize, ability to work with a diverse population, and ability to work independently and as part of a team.
- Two years of experience in the field or lived experience with substance use or homelessness preferred.
- Reliable attendance and punctual reporting of work required.
- Previous training in crisis prevention/intervention strongly desired (i.e., CPI or equivalent).
- Valid driver’s license required.
- Bilingual, multicultural, and candidates with lived experience/expertise are encouraged to apply.
PHYSICAL REQUIREMENTS
Ability to operate a motor vehicle; climb stairs; possess good communication skills; meet the requirements of the classification and have mobility, vision, hearing and dexterity levels appropriate to the duties to be performed. Capable of the following extended activities: standing, sitting, climbing stairs. Capable of the following intermittent activities: lifting 25 pounds or more and bending in the performance of infrequently performed office duties.
