Program Description
The CES North County Homeless Resource Center is a centralized hub providing housing services, support and referrals for people who are homeless and at risk of homelessness in Berkeley, Albany and Emeryville. Staff assess and triage people’s housing needs and people who are literally homeless will be offered an array of services to meet emergency housing needs and to address their longer-term housing needs.
Project Hope: Since July 2013 Insight Housing has been providing outreach and Housing Navigation to homeless residents of the City of Albany. The Albany Housing Navigator conducts street outreach and engagement activities to participants who are still on the street and assists with housing search/landlord relationship development, and housing placement. The Albany Housing Navigator also conducts home visits to households of formerly unhoused people to provide intensive case management services. The job requires the ability to work across the entire spectrum from outreach to housing retention and use models of Harm Reduction, Housing First, Trauma Informed Care, and Progressive Engagement. 
 
Essential Duties and Responsibilities
  • Provide outreach and engagement to people who are unhoused in encampments, in front of businesses, parks, shelters, and other locations
  • Conduct home visits to participants who are housed
  • Assist clients in meeting basic needs for food, shelter, medical /mental health care, ID’s, etc
  • Conduct CES assessments of each participant every 6 months and create individual housing stability plans every 90 days for all engaged clients
  • Complete enrollment in HMIS within 24 hours of acceptance of services (outreach, navigation, Rapid Re-Housing)
  • Responsible for setting administrative time to enter case notes at least once a month; may require weekly case notes for outreach depending on the participant needs
  • Maintain and track documentation of outreach efforts if participants don’t agree to enroll in a program right away (i.e. in a Word document, Excel tracker form)
  • Carry an intensive caseload of 10-15 high needs participants
  • Provide ongoing housing search activities
  • Locate and build relationships with landlords who are willing to house homeless Albany clients
  • Match landlord qualifications with client needs
  • In collaboration with the Program Manager, negotiate rental agreements with landlords, conduct housing inspections, and assist participants with review of lease agreements and move-ins
  • Complete paperwork and obtain approval from Program Manager/Management for rapid re-housing subsidies for clients as needed
  • Provide monthly home visits to Albany households receiving a rapid re-housing subsidy.
  • Maintain collaborative working relationship with the City of Albany and other service providers
  • Prepare monthly and quarterly statistical and narrative program reports
  • Provide accurate participant information, new outreaches made, and other information as needed by Program Manager to complete monthly and quarterly reports.
  • Work collaboratively with all City of Albany staff including staffing the resource center two days a week.
  • Provide Medi-Cal outreach
  • Engage in referrals, coordination and monitoring of Medi-Cal Services (outreach, completing applications for insurance and related services)
  • Attend MAA/TCM Implementation Training
  • Drive own or agency vehicle to various sites as required; documenting and reporting mileage according to agency procedures, so that services can be provided in a timely manner; comply with agency driving policy at all times. 
  • Participate in promoting a safe, healthy, and clean working environment consistent with agency’s health and safety practices.
  • Attend and participate in all meetings and trainings as assigned. 
  • Complete and submit timesheets in a timely and accurate manner.   
  • Work within the framework of Insight Housing’s Code of Conduct. 
  • Perform other tasks as assigned. 
 
Qualifications, Skills, and Abilities 
  • High school required. Bachelor’s degree in Social Services or related field a plus.
  • Ability to complete CPR/First Aid Training.
  • 1 year experience providing direct support services to individuals experiencing homelessness, mental health, substance abuse or other social issues, required. Translatable relevant internship and volunteer experience or administrative experience will be considered. Prior Customer service, mentorship, administrative, extensive documentation, or experience providing life training skills or eligibility advisement in military will be considered.
  • Experience with Rapid Re-Housing required
  • Experience with housing retention programs preferred
  • Commitment to serving individuals experiencing homelessness, alcohol and drug dependency, and other mental health issues and to trauma-informed approach to care of individuals. 
  • Lived experience strongly preferred.
  • Proficient in use of computer and Microsoft office suite (Word, excel, outlook and TEAMS). Experience with Homeless Management Information System (HMIS) a plus
  • Ability to interact in a supportive and professional manner with staff and clients of diverse cultural and economic backgrounds. 
  • Ability to provide good customer service. Communicate effectively and maintain a calm demeanor in stressful situations.
  • Capable of working independently and as part of a team.
  • Excellent verbal and written communication skills. 
  • Excellent interpersonal and crisis intervention skills with a can do and flexible attitude. Ability to work well under high pressure. 
  • Ability to work with minimal supervision, multitask, maintain confidentiality and appropriate professional boundaries, and meet deadlines. 
  • Ability to maintain professional conduct, attitude, and appearance at all times. 
 
Special Requirements 
  • This position requires frequent driving.  A valid California driver license, reliable personal vehicle, current personal auto insurance as required by law, and an MVR sufficient to obtain and reasonably maintain insurability under agency auto liability policies.  
  • Must be able to receive and maintain criminal records clearance.
  • Must be able to obtain TB clearance.
 
Physical Requirements  
  • Regularly required to use hands and fingers to handle, feel, or operate objects, tools or controls, and reach with hands and arms.  
  • Frequently required to stand, walk, sit, climb stairs, talk, hear, and see clearly.  
  • May be occasionally required to stoop, kneel, or crouch.  
  • May be required to lift or move up to 50 lbs.
