Outpatient programs provide trauma-informed care aimed at reducing the harmful impacts of substance use. Participants and their counselors work collaboratively to design an integrated, individualized, strengths-based treatment experience geared towards addressing each client’s specific needs and goals. Outpatient programs provide Individual and group therapy, individualized case management, treatment planning, drug and alcohol counseling, crisis intervention, and connection to social support services.

The AARS Adult outpatient substance use treatment program (ASATS) specializes in providing culturally responsive services in several threshold languages like Vietnamese, Spanish, Ilocano, and Mandarin. ASATS provides individual counseling and an array of counseling groups that focuses on specific challenges like co-occurring disorders, criminal-justice impact, and many more.

Hours of operation are Mondays and Fridays from 9:00AM to 6:00pm; Tuesdays, Wednesdays, and Thursdays from 9:00AM to 8:00PM.

The Care Coordinator II is an integral part of an interdisciplinary team that supports our clients through assessment of their needs and strengths in support of their health and recovery. Care Coordinator II will perform counseling in individual and group setting, facilitate group sessions including process groups, psychoeducational groups, relapse prevention, deliver curricula in accordance with program specific requirements and case management, and assist clients in navigating systems of care while providing a supportive treatment environment. The Care Coordinator II provides substance use individual and group therapy based on individualized assessments. The intensity of services is based on the client's stage of change and response to treatment. KEY RESPONSIBILITIES

Individual Treatment Responsibilities:
  • Facilitates intakes/orientations, comprehensive assessments, individualized treatment planning, crisis, collateral, case management and individual counseling sessions with clients.
  • Provides learning experience opportunities and offers clinical support to assist clients in meeting their treatment goals.
  • Proactively links clients to both internal and external resources based on their treatment needs and follows up on the progress/status.
Treatment Setting Responsibilities:
  • Facilitates group counseling and education sessions as main modality of treatment.
  • Conducts community outreach and education workshops.
  • Performs crisis intervention and works with treatment team to stabilize crisis situations.
  • Attends required trainings and meetings.
  • Plans and facilitates client celebrations and special events.
  • May be asked to work on a weekend or during after-hours for crisis stabilization, community education, resource fairs, and special events.
  • Will work at agency’s main office and in addition to co-location sites which may include schools, community health centers, in-home, and juvenile detention centers.
Documentation Responsibilities:
  • Collaborates with each caseload client and other available internal and external resources to develop/maintain treatment plans, transition plans, progress notes and appropriate updates in support of the health and recovery needs of the client.
  • Properly documents all individual and group counseling sessions, attempts to contact, and completes the discharge paperwork/process and required agency assessments in timely manner.
  • Also, maintains accurate records by data entering documentation into various electronic systems for all caseload clients in accordance with guidelines established by HealthRIGHT 360 to satisfy internal and external evaluating requirements.
  • Will participate in Peer Chart/ Utilization Reviews for preparation of program audits. Will adhere to documentation policies and procedures set forth by Compliance Department.
  • Be responsive and pro-active in all correspondence with referring parties, judges, probation and parole officers, social workers, and the Quality Improvement Dept. via treatment status reports, phone calls, faxes, etc.
  • Will provide informed, written status reports to the above entities as appropriate.
  • Will assist in providing written progress reports/narratives to Clinical Program Manager or Directors as requested.
Productivity and Coverage Responsibilities:
  • Must achieve 50% of service delivery goals while delivering high-quality care and services.
  • May be required to meet service delivery goals at other Santa Clara County Behavioral Health programs.
  • Ensures coverage during absences or unplanned events by coordinating with team members and management.
  • Adjusts workload as necessary to ensure uninterrupted service delivery, including stepping in to provide support during peak demand times or staff shortages.
  • Adheres to the program’s scheduling protocols to maintain appropriate client-to-provider ratios.
QUALIFICATIONS:
Education, Experience and Certifications
  • Registration or Certification in a Drug and Alcohol program recognized by DHCS.
  • First Aid Certified within 60 days of employment.
  • CPR Certified within 60 days of employment.
  • Valid California driver’s license.
  • Extensive experience with substance abuse treatment preferred.
  • Experience working in schools or in juvenile detention centers preferred.
  • Experience working within the Santa Clara County –Department of Behavioral Health Services system preferred.
Background Clearance
  • Must be able to complete a background check and livescan clearance
Knowledge
  • Knowledge of substance abuse recovery principles and resources; including Co-occurring disorders and trauma informed treatment.
  • Knowledge or certification with the Seven Challenges and other evidenced based models encouraged by Santa Clara County Department of Behavioral Health Services preferred.
  • Knowledge of cultural stigmas of counseling and treatment services.
  • Knowledge of principles and techniques of social work, case management, crisis intervention, client centered counseling, and group facilitation.
  • Knowledge of child development.
  • Strong proficiency with Microsoft Office applications, specifically Word Outlook and internet applications.
  • Knowledge and experience with Medi-CAL documentation standards preferred.
  • Knowledge of mental health recovery and evidence-based practices preferred.
  • Bilingual Skills: Proficiency in one of the County’s threshold languages (e.g., Spanish, Vietnamese, Tagalog, Mandarin/Cantonese) is highly preferred for effective communication with diverse populations.
