• Provides telephonic outreach to patients with orders
• Assist in identifying and closing gaps in care
• Collaborate with healthcare team to optimize patient adherence to medical plan of care
• Educates patient and family members about preventative screenings while identifying barriers to care
• Interacts with healthcare team
