What are the responsibilities and job description for the Care Manager position at HEALTHSOURCE OF OHIO?
HealthSource of Ohio is seeking to hire a Care Manager who is passionate about providing quality healthcare to our patients and communities. We are looking for Friendly, Accessible, Credible individuals to join a great team.
Responsibilities:
- Assists patients through the healthcare system by acting as a patient advocate and navigator.
- Coordinates continuity of patient care with patients and families following hospital admission, discharge, and ER visits.
- Educates patient and family on ways to reduce ER visits in the future
- Participates in Patient-Centered Medical Home team meetings and quality improvement initiatives.
- Evaluates clinical care, utilization of resources, and development of new clinical tools, forms, and procedures.
Qualifications:
- Current RN Licensure in the state of Ohio
- BA in nursing preferred/ Associates required
- 2 to 3 years clinical practice experience
- Well-rounded clinical experiences including work in outpatient settings Experience with data analysis and health care operations
