Position Summary:
The Outreach Educator’s role is to strategically identify and target at-risk communities with pregnant women who lack the resources to receive early and consistent prenatal care. The Outreach Educator must remain committed to locating and linking pregnant women into prenatal care, with a focus on women at greater risk of poor birth outcomes. This position also provide education to the community through presentations, printed materials and classes/workshops. Local travel required.
Essential Functions and Responsibilities:
Develop and implement strategies to promote the HMHB programs and services to the community to increase engagement, both internally and externally.
Provide direct community outreach and health education to identify and refer pregnant women to HMHB and system services through events, grassroots efforts and group presentations.
Utilize networking skills with businesses, health entities, religious organizations and others to build community relationships, identify and eliminate any potential barriers to receiving prenatal care and expand outreach efforts.
Conduct educational presentations on health-related topics and social determinants of health at community locations.
Facilitation of HMHB classes/workshops, inclusive of coordinating logistics, conducting surveys and collecting data from participants for reporting purposes.
Promote Healthy Beginnings programs and system partners as coordinated system of targeted maternal health services.
Utilize both internal and external data systems to track client referrals and activities.
Attend community and other relevant events and meetings that impact the achievement of the program objectives. Provide information on resources for pregnant women in Palm Beach County.
Attends pertinent meetings, trainings and conferences related to the position and as deemed by Supervisor.
Responsible for program activities that align with and ensure program outcomes are met. Provide monthly activity reports and other data as requested. Complete paperwork as needed programmatically and administratively.
Knowledge, Skills and Abilities:
Excellent interpersonal, relational, problem solving and communication skills; both verbal and written.
Computer skills including Microsoft Office Suite.
Excellent presentation skills.
Considerable knowledge of principles and practices of working with diverse populations.

Bilingual preferred.
Experience Required:
Minimum of one year of experience working in a social service setting.
Experience in customer service.
Demonstrated experience with presentation skills preferred. Demonstrated flexibility and ability to work in various roles, settings and capacities while respecting boundaries in each role.
Educational Requirements:
High School Diploma or equivalent.
