Schedule: Part time day and evening shifts available. Includes every other weekend.

The Licensed Practical Nurse (LPN) is responsible for the provision and monitoring of high-quality clinical care and services to residents consistent with regulations and established clinical best practices.

The LPN reports directly to the Clinical Coordinator and may take direction from the RN on duty. The LPN monitors the work of Medication Assistants and Resident Assistants in the community on his/her assigned shift.

Responsibilities:

1. Maintain an environment that achieves high quality care (physical, mental, emotional, social and spiritual) of residents and maintain their independence and dignity to the highest degree possible within the Heartwood continuum. Maintain effective communication systems with all customers.

2. Follow the steps of the nursing process as stated under the nurse practice act under supervision of the Registered Nurse: observation, planning, implementation, and evaluation of nursing care based upon regulations and established clinical best practices. Perform nursing techniques for the comfort and well-being of each resident. Maintain all required documentation in accordance with regulations and established clinical best practices.

3. Promote a spirit of cooperation with all regulatory organizations managing with an expectation of appropriate compliance and corrective measures. Coordinate clinical care with other health care professionals inside and outside Heartwood.

4. Maintain a work environment for employees conducive to high productivity and morale. Maintain effective communication systems with employees. Support employee education and development opportunities which meet regulatory requirements and opportunities for growth.

5. As a member of the clinical and community team, actively participate in team activities, meetings and processes. Assist in the orientation of new team members as assigned. Represent clinical issues at community functions. Effectively collaborate with staff to promote the residents’ total well-being.

6. Monitor the performance of assigned staff under supervision of the Registered Nurse. Coach and assist staff to achieve desired outcomes and individual development.

7. Implement established clinical policies, procedures and practices. Ensure policies and procedures are understood and implemented by assigned staff. Use quality management process to meet the needs of residents, staff and clinical services in the community. Maintain monitoring systems and processes to ensure compliance with regulations and established clinical best practices. Apply the knowledge and skills needed to do the position, including technical competence in the clinical field; maintain knowledge of organization and industry policies and practices

8. Assist with implementing and managing staffing schedules in the community that meet resident care needs and financial goals. Demonstrate good stewardship of available resources (supplies, utilities, time).

Qualifications


1. Licensed Practical Nurse with current licensure with the State Board of Nursing in Minnesota.

2. Interest in gerontological nursing, rehabilitative and restorative clinical practices.

3. Demonstrated compatibility with Heartwood’s mission and operating philosophies.

4. Demonstrated ability to read, write, speak and understand the English language to communicate with all customers.
