SUMMARY OF JOB RESPONSIBILITIES:
Responsible for leading and managing the coordination, planning and day-to-day leadership of patient care and operations provided by the nurses, home health aides, social workers, chaplains for Centrica Hospice and Hospice of Michigan SW in compliance with all regulatory standards and agency policies. Develops, plans, implements, and evaluates nursing, home health aides, social workers, and chaplains to ensure the quality and safe delivery of care, treatment, and services.
SUPERVISES: Leaders of Nurses, Social Workers, Chaplains, Home Health Aides, and Clinical Assistants
QUALIFICATIONS & EXPERIENCE:

A bachelor’s degree in science in Nursing required. Master’s degree in nursing or healthcare administration or related field preferred.

Registered Nurse in the state of Michigan.

Experience working in or leading clinical teams and operations with strong clinical acumen and experience delivering patient care.

Three (3) to five (5) years of progressive leadership experience in a related healthcare organization with preferred experience in clinical operations.

Experience with palliative, end-of-life, home, or community-based health care preferred.

Experience working with multi-disciplinary teams, building consensus, and leading people and projects forward.

Present a negative TB skin test and/or chest X-ray and other tests as required by the agency’s policy.

Maintain a valid Michigan driver’s license and has the availability of a reliable, licensed, and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.

Remains up-to-date on all routine immunizations required by LARA and agency policy.
