Job Classification: Stepping Up Case Manager
Working Title: Stepping Up Case Manager – Blount County
This Case Manager III position works as a direct service provider and an information and referral agent for members of the target population, individuals who have a serious mental illness and have frequent interactions with jail admissions. The role of the Stepping Case Manager is to reduce the recidivism individuals have historically experienced by serving as a bridge to community resources.

Division: Outreach
Normal Work Hours/Days: 40 hours per week - Monday through Friday. Flexibility required for varied work hours based on program need and individual client requirements. Some evening and weekend hours work will be required.
Primary Job Functions:
• Perform Brief Jail and Uncope screening assessments of inmates as referred by Blount County jail personnel
• Provide access to a wide range of services for individuals experiencing serious mental illness including direct services.
• Work collaboratively with Blount County jail and court staff
• Properly assess needs and devise individual service plan for each individual
• Effectively implement written service plans to meet individual’s goals and objectives.
• Serves as liaison between the individual and various service providers to ensure continuity of care.
• Refer inmates for clinical assessment to local community mental health center
• Assist SMI/SU screened inmates with reintegration to community
• Provide community-based services to released SMI/SU individuals
• Develop and maintain adequately detailed and accurate individual records
• Maintain grant required data in designated database and enter service data in EMR
• Provide transportation to individuals when necessary
• Provide accurate documentation in debriefing for weekly and monthly reports/meetings as stipulated by grant/program demand. This will include at minimum a monthly collaborative meeting, monthly technical assistance CM meeting, quarterly technical assistance meeting, monthly planning team meeting, weekly treatment team meetings, Mental Health Court, and conduct ‘May Month of Action” activity recognizing Stepping Up and Mental Health
• Assist in the completion of mid-year and final year reports
• Perform all administrative duties as required.
Qualification:
• Bachelor of Arts or Bachelor of Science in a human service-related field. Two years of experience working with individuals living with mental illness.
• Valid Alabama driver’s license and driving record acceptable to agency automobile insurance carrier for inclusion or agency policy.
• This position requires completion of the State of Alabama Adult Serious Mental Illness Case Management Training.
Knowledge, Skills, Abilities:
• Knowledge of mental illness, symptoms, and treatment regimens.
• Ability to work autonomously and make sound judgements regarding individual care.
• Knowledge of the special needs and behavioral characteristics of individuals experiencing serious mental illness.
• Knowledge of the Blount County community and available community resources
• Knowledge of the rules and regulations governing the human service provider agencies in the catchment area.
• Knowledge of the legal and ethical issues relative to confidentiality of individual records and the ability to interpret and apply them within the scope of the position.
• Ability to communicate effectively with a multidisciplinary team.
• Ability to document activities and maintain records in a timely fashion.
• Skills in interpersonal relationships and dynamics.
• Considerable written and expressive communication skills.
