JOB TITLE: Case Manager
JOB CLASSIFICATION: Mental Health Worker 2
REPORTS TO: Project Supervisor/Program Director
SUMMARY:
Beginning professional work managing cases of serious mentally ill or co-occurring substance use individuals who have been incarcerated in the local jail, been admitted to a local psychiatric hospital, or sought psychiatric treatment at a local hospital emergency department. Work includes, for each individual case management services, human service needs assessment, case planning, and follow-up. Individuals residing or located in the Cleburne County area is the focus population for this position.
EXAMPLES OF DUTIES AND RESPONSIBILTIES:
1. Linkage-the interface of the individual with the appropriate person or
agency necessary to implement the case plan.
2. Advocacy-calls and/or visits a person or agency on the individual’s behalf
and follows up to determine that the individual is receiving needed services.
3. Crisis intervention-assists the individual through crisis situations and/or
arranges for the provisions of such assistance by other professional/
personal care givers.
4. Transportation-assures that the individual is aware of location of service and/or arranges for transportation if individual is unable to provide own transportation.
5. Completes all required administrative and clinical paperwork.
6. Active monitoring and encouragement to maintain active engagement with service recommendation(s).
7. Participates in staff meetings.
8. Participates in pre-release case planning for all appropriate institutional
patients being prepared for return to the catchments area.
9. Responsible for the storage and protection of consumer records.
10. Performs other related duties as required.

1. Ability to communicate verbally and in writing with individuals , family members, staff and outside agencies.
2. Some knowledge of mental illness and substance use.
3. Knowledge of community resources and services. .
4. Possess to high degree of organizational skills.
5. Must demonstrate the ability to work well with staff and individuals in a polite, respectful and dignified manner.
QUALIFICATIONS:
Bachelor's Degree in a social service related field from an accredited college or university.
Case Management Certification by the Alabama Department of Mental Health and Mental Illness.
Stepping Up Certification.
Valid Alabama Driver's License.
