Reporting directly to the VP, Jefferson Primary Care (JPC) and collaborating with the Enterprise Chief of Primary Care, the Director of Primary Care Centralized Services is responsible for operational excellence across all JPC Centralized Services including Supplemental Access, Clinical Risk Assessment, Integrated Behavioral Health, and Annual Wellness Visits, and other centralized services as this portfolio evolves. Specific responsibilities include, but are not limited to:
1. Operational leadership for Primary Care Centralized Services, with programs including Supplemental Access, Clinical Risk assessment, Integrated Behavioral Health, and Annual Wellness Visit programs.
2. Implementation of high-quality clinical care and excellence in patient experience across all JPC Centralized Services.
3. Accountability for budgeting, for financial and operational monitoring, and for meeting budgeted goals across Primary Care Centralized Services including both traditional and value-based business models.
4. Developing and evolving new services and care models and bringing them to scale.
5. Convening and engaging key stakeholders across organizational units for design, implementation, evolution, and evaluation of clinical programs.
6. Identifying and monitoring key performance indicators for new and evolving clinical programs.
7. Working effectively across an interprofessional and matrixed environment. The director serves as a core member of the JPC leadership team and will support ongoing growth and infrastructure development as requested.
Qualifications
Education:
BA required
Masters strongly preferred (MBA, MPA, MPH, Health Administration)
Experience:
Minimum 5-7 years of related progressive health care management experience, preferably in a Primary Care or related setting
