Description
RESPONSIBILITIES: The OT is responsible for completing evaluations, writing functional goals, and implementing treatment plans, as well as submitting the necessary documentation to obtain DHS funding for individuals in the program.
SPECIFIC DUTIES:
- Must be capable of speaking English clearly and fluently to communicate with staff, families, and individuals. Must be capable of writing English in a clear, concise, grammatically correct, and free of spelling errors. In the event the individual/family/staff etc. does not speak English, an interpreter will be utilized.
- Must act in a professional manner that reflects positively on themselves and Little Friends Inc./Spectrum Vocational Services. This includes but is not limited to demonstrating high commitment to individual services and satisfying all Little Friends, Inc/Spectrum Vocational Services internal and external customers.
- Is aware of and follows the policies and procedures of Little Friends, Inc. /Spectrum Vocational Services.
- Must develop and write treatment plans. This includes but is not limited to:
- Completing OT evaluations
- Writing and tracking measurable goals monthly, or as needed
- Consult with all members of the treatment team to ensure comprehensive services
- Obtain guardian consent to contact references/resources such as physicians
- Provide direct, habilitative services to individuals on caseload as needed.
- Complete necessary DHS 52O applications to obtain funding for individuals through the Medicaid waiver.
- Work cooperatively with Program Managers and QIDPs to ensure services are reflected in the Personal Plan/Implementation Strategy.
- Maintain documentation, such as SOAP notes, to document billing hours.
- Complete monthly ROCs billing and submit to the program secretary by the 5th billing day of every month.
- Keep credentials current and maintain necessary paperwork/documentation.
OTHER DUTIES:
- Consult with families regarding integration of occupational therapy plans into the home environment, as necessary.
- Work cooperatively with Residential Services to provide in-home occupational therapy evaluations and write home recommendations.
- Complete annual summaries of services and progress.
- Act as a site mentor for future Occupational Therapy students/interns.
- Other duties as assigned by the Vice President of Adult Day Services
Requirements
- Master’s or Doctorate Degree in Occupational Therapy with authorization to practice as an occupational therapist in the state of Illinois
- Evident ability to make sound judgments
- Possesses the ability to develop and implement appropriate treatment plans.
