JOB SUMMARY:
This position is on AccessHealth Horry’s BCN (Best Chance Network) team, which is funded thru a contract with SCDHEC for the purpose of engaging women who meet BCN (Best Chance Network) program standards back into care for their preventive screening to decrease the risk of cervical/breast cancer. The Patient Navigator serves as the liaison position between patients and community healthcare providers to ensure continuity of care for those patients who are or have been participants in the Best Chance Network (BCN) program.
Patient Navigator will work with identified lists of women from DHEC who are or were enrolled in the BCN program and have been lost to care or have been followed up within the past 24 months or longer. Patient Navigator will be responsible for documenting all contacts into SCDHEC’s EMR system, educating local healthcare providers about the BCN program and goals, and assist in recruiting new agencies to become BCN providers.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following but are not limited to:
- Exhausts outreach measures to establish communication with women who are either lost to care or need assistance with the follow up back into the BCN program.
- Assists with scheduling of BCN appointment, education, transportation, and provides case management follow up.
- Secures the necessary documentation to determine eligibility into the BCN program.
- Serves as an advocate for BCN patients and promotes the overall goals and mission of the BCN program.
- Documents and maintains complete and accurate patient records on all eligible applicants into SCDHEC’s BCN program.
- Serves as a member of the AccessHealth Horry team to promote, educate and inform patients and staff of available community resources and programs.
- Performs miscellaneous job-related duties as assigned.
- Reports directly to the BCN RN Supervisor.
QUALIFICATIONS:
- Strong interpersonal and communication skills and the ability to communicate effectively with patients and providers, both verbally and in writing.
- Demonstrate a strong and positive desire to want to help others.
- Strong organizational and time management skills.
- Ability to document and maintain patient’s information in an EMR system.
- Ability to gather data, compiles information, and prepare reports.
- Ability to coordinate and organize diverse program materials.
- Must be a team player and work well with other staff members.
- Ability to work efficiently and effectively in a remote working environment.
- Effective verbal and written communication skills.
- Strong ethics and the ability to work with minimum supervision.
- Must have a valid driver’s license and provide own transportation.
- Ability to speak Spanish is preferred, but not required.
EDUCATION and/or EXPERIENCE:
- Bachelor’s degree or High School Diploma
- At least 1 year of experience in the human services field; OR at least 2 years of experience that can be demonstrated to be applicable to the duties listed in the job description.
