Job Description

As a Care Navigator, you will play a crucial role in supporting our end users, primarily older adults with memory loss, anxiety, or agitation and their caregivers, in utilizing our Resonance Rx application. Resonance Rx is a Remote Therapeutic Monitoring application that qualified healthcare providers, often Psychiatrists and Psychologists, use to manage and monitor cognitive and behavioral issues in their patients. You will support end-user success by identifying each patient's needs and implementing steps to guide improvements in adherence and outcomes.

The candidate should employ effective methods to guide end-users through their journey and build strong relationships with caregivers.

Responsibilities

• Onboard end-users onto the Resonance Rx platform over video or phone call.
• Provide part-time remote monitoring services to help support users as they progress through their cognitive behavioral and music therapy program at home.
• Direct and aid users in their use of the Resonance Rx platform and home exercises, including technical support.
• Provide periodic feedback to the evaluating clinician on user progress.
• Maintain necessary records and notes while providing remote monitoring.
• Assist with improving processes and functionality of the platform.

Requirements

• A resident of California with an active RN license in California.
• As a Registered Nurse, you are naturally compassionate, cordial, and respectful toward those in your care.
• Tech-savvy with a modern laptop, reliable Wi-Fi, and a smartphone.
• 2 years of experience in geriatric care or hospital nursing.
• Experience with remote work and/or telehealth.
• Experience in behavioral modification; Health Coach Certification is a plus.
• Excellent communication, interpersonal, and presentation skills, with the ability to collect and present meaningful data.
• Strong relationship-building and listening skills.
• User-focused with a genuine passion for enhancing user experience.
• Capable of multi-tasking and managing diverse user needs.
• Able to work independently with minimal supervision.
• Flexible and agile in a fast-paced, cross-functional environment.
• Skilled at influencing change and bringing enthusiasm to the workplace.
• Independent, proactive, quick thinker with an open and inquisitive approach.
• Eager to make an impact in a young company addressing a significant issue.
• Must love music!

Compensation

• Competitive hourly rate.
• Bonus opportunities based on end-user satisfaction and success.

Job Types: Part-time, Contract

Pay: $30.00 - $40.00 per hour

Expected hours: 4 – 20 per week
