Community for Life, Wicomico leverages community volunteers to support older adults who wish to remain living independently in their homes. Members receive support with transportation, home maintenance, advocacy, social engagement, and unlimited service navigation. The program is designed to allow seniors to enjoy their independence and quality of life in the home, for years to come.
JOB DESCRIPTION
The service navigator will support the CFL Director to assist in the implementation of the Community for Life program. This position will be instrumental in delivering high quality service to members of the program and responsible for recruitment and intake of new members to the program. This role will be pivotal in the expansion of the membership of CFL, through active recruitment and onboarding of new members.
The Service Navigator will ensure that member requests are collected and addressed by volunteers in a timely fashion provide members with services including: transportation assistance, service navigation, and home maintenance. This role will provide assistance to the CFL Director and others to plan and implement social engagement activities, that include (but are not limited to) social and educational events, holiday celebrations, and volunteer recognition programming.
KEY RESPONSIBILITIES
The Service navigator will:
· Provide high quality social interaction with each member, conducting outreach touchpoints to members on a regular basis.
· Complete Home Maintenance and Safety Assessment for all new members.
· Conduct bi-weekly home visits as needed to members, helping in the home.
· Identify opportunities for outreach to new members to grow the program.
· Support the program marketing and outreach efforts.
· Maintain member financial records, as needed.
· Maintain records of services provided to each member, identifying service area strengths and potential gaps to service.
· Communicate all member needs to the CFL team to ensure volunteers are assigned and able to meet the needs of each request.
· Work in collaboration with program team and volunteers to implement social activities for all members, including large event planning and informal social interaction activities.
· Maintain all documentation related to the program including client files and referral data.
· Provide reports as needed on client outcomes, number of clients served, demographic data, etc.
· Assure participant’s personal information is handled in accordance with privacy statements and policies.
Ideal candidates:
· Are fluent with Microsoft Office Applications (Word, Excel, Outlook, etc).
· Excellent communicators, providing exceptional customer service to every member, with a patient and caring approach to service for senior citizens.
· Hold a valid Maryland driver’s license and ability to transport self and members as needed.
· Bachelor’s Degree with minimum 2 years’ experience working with senior citizens, OR an Associate’s Degree with minimum of 5 years’ experience working with senior citizens.

Pay: $18.00 per hour
Expected hours: 20 per week
