Healthcare Coordinator Overview
Responsible for the direction and overall delivery of healthcare and medical services for the ICF / ID Programs. Gives direct supervision to all Primary Care Nurses (PCN) and shift nurses in the ICF / ID Programs.
Coordinates all the systems for the medical service and health care needs of the individuals receiving services at the Melmark Campus Intermediate Care Facility for Individuals with Intellectual Disabilities(ICF / ID).
Duties and responsibilities include the provision of excellent healthcare services for all residents receiving services in the Campus ICF / ID Programs 24 hours a day.
Schedule and Salary Information
- Monday- Friday 8a-4 : 30p
- On call rotation and holidays as assigned
- Starting rate with LPN : $33.00 ( ) based on years of experience
- Starting rate with RN : $43.00 ( ) based on years of experience
Requirements
- Current PA nursing license required. Registered (RN) Nurse highly preferred. Licensed Practical (LPN) Nurse eligible.
- Must hold a US Driver’s License and must be able to meet requirements to become an approved Melmark driver. the position
- Knowledge of regulatory standards for PA’s DPW 6600 Regulations.
- If RN, two years of direct practice experience required. If LPN, five years of direct practice experience required. Experience in supervising employees highly preferred.
Healthcare Coordinator Responsibilities
Supervises the Primary Care and shift nurses in the adult areas. Supervises and is responsible for all nursing tasks, including medication administration (training and oversight) and all medical aspects.
Provides in-service for all employees and individuals regarding health care issues.
- Ensures all required medical paperwork is completed (90-day reviews, Utilization Reviews, HRT reports, etc). Provides regular reviews of medical records (appointments, lab results, current proper diagnosis)
- Facilitates means and methods by which nursing personnel can work with groups in interpreting the goals of Melmark for each individual, their family and the community.
Protects and promotes individual rights; including confidentiality, privacy, dignity and freedom from abuse.
In conjunction with the PCN, coordinates with Primary Care Physician (PCP) for proper medical care and wellness of all individuals.
Identifies and ensures treatment of communicable diseases and reports all infectious conditions to appropriate personnel.
- Attends weekly management meetings. Attends weekly IDT meetings. Attends HRT and ISP meetings.
Frequent interface with doctors, nurses and practitioners.
