We are looking to hire a committed, dependable, and task-oriented community Stabilization Counselor who will make thoughtful connections with our clients.
The Crisis/Community Stabilization Counselor provides direct mental health care to individuals experiencing a mental health crisis that may put their current community living situation at risk. This service provides an emergency response to situations where an individual’s behavior causes a concern for safety. The objective is to deter hospitalization by maintaining clients safely in the community.
Qualifications: Candidates must have a masters degree in a human services field (e.g. Psychology, Sociology, Social Work, etc.).
Candidates must maintain LMHP (LPC or LCSW status with the Virginia department of health professions regarding education, experience and continuing education. **Possessing only QMHP does not qualify for this role.
Must have working knowledge of the areas of need for adults with serious mental illness; must have appropriate clinical skills to facilitate training in daily living skills to adults with serious mental illness; must be able to work from a person-centered framework
Must be able to identify clinical risk factors for suicide, violence and other at-risk behaviors.
Duties and Responsibilities:
· Provide intensive crisis stabilization services in the client’s residence and/or community at a frequency based on the need of the individual.
· Provide goal-directed short-term counseling interventions to stabilize an individual’s mental health condition and prevent the individual from being at risk for hospitalization due to psychiatric instability.
· Monitor psychiatric symptoms and refer individual for crisis intervention evaluation/inpatient admission if symptoms intensify to the degree the individual is at risk for imminent harm to self or others.
· Monitor medication adherence and ensure individual completes a psychiatric evaluation during crisis admission.
· Refer client for community resources and professional mental health and medical supports needed to stabilize their condition.
· Collaborate with wraparound service providers through service coordination efforts that is documented in the client record.
· Complete daily progress notes, discharge reports, service authorization reports, incident reports and other case documentation according to agency procedures.
· Ensure that service documentation is individualized based upon the needs and progress of the individual.
· Attend clinical supervision with Clinical Supervisor at a time designated by the Supervisor.
Schedule: Varies based on client needs. On call required.
