What are the responsibilities and job description for the Homeless Shelter Case Manager and Peer Recovery Coach position at OpenDoors?
Case Management
Provide shelter based or transitional housing based case management to guests staying in OpenDoors shelters or transitional housing by working with clients to both maintain stability in the shelter and obtain permanent housing. OpenDoors provides support to people that are struggling with homelessness, addiction, and previous criminal records. OpenDoors runs a variety of facilities that work with similar populations, and this position will be asked to work in different locations.
Conduct an initial assessment and help the client develop a plan that covers each life area, including housing, employment and recovery.
· Follow all Case Management duties, as laid out in the Case Management Protocol document, which involve providing support around housing, employment, training and education, probation and parole supervision, court issues, financial planning, transportation, addiction and mental health issues, and family and social support issues.
· Provide peer recovery coaching services and complete all Medicaid-required paperwork. If not already certified as a peer recovery coach, pursue certification.
· Provide Housing Stabilization Services and complete all Medicaid-required paperwork.
· Meet at least once a week in a one on one session with each client
· Facilitate the client towards becoming independent financially and in their recovery, and assist them in developing a personally directed, community directed recovery with multiple supports.
· Track client progress with case notes and records and reports as needed.
· Assist participants to access community resources, schedule and attend appointments.
· Maintain professional rapport with service user, peers, and other community service providers.
· Uses assessment information to establish client centered goals, develop service plans which contain specific measurable objectives and timeframes for completion.
· Communicates with internal and external departments regarding case coordination and complete safety plans and risk minimization to appropriately support clients/residents.
· Provides referrals to appropriate other service providers within the agency and externally.
· Balances the needs of caseloads based upon acuity, length of time in the program, state of housing stability, etc., supporting 20-30 individuals at one time.
· Advocates for the client/residents as needed.
· Reviews and evaluates resident progress on an ongoing basis and adjusts case plans as appropriate.
· Attends all required trainings and participates in voluntary trainings as needed.
· Maintain all administrative duties including but not limited to: maintaining current documentation for program eligibility and compliance, electronically and on paper as well as documentation related to time and effort.
· Other duties as assigned.
Required Work Experience
Minimum 2 years experience work as a housing case manager with similar population.
Preferred Experience
- Bachelor’s degree in Human Services field OR
- Associates Degree in a Human Services field plus a minimum of 2 years experience in a Human Services field such as advocacy or case management OR
- A certification in Case Management plus a minimum of 2 years experience in a Human Services field such as advocacy or case management or peer recovery coaching certification OR
- A minimum of 8 years experience in a Human Services field such as advocacy or case management.
Job Type: Full-time
Pay: $21.00 - $24.50 per hour
