Position Details:
- Main Office location: Stoneham, MA
- Service Area: Stoneham & Lawrence, MA Territories
- Job Type: Full-time
- Schedule: Monday – Friday
Responsibilities:
- Initiate appropriate preventive and rehabilitative nursing procedures.
- Administer medications and treatments as prescribed by physicians.
- Conduct initial assessments of patients and families to determine home care needs.
- Perform thorough physical assessments and gather comprehensive medical histories.
- Develop and revise individualized plans of care based on patient status and evolving needs.
- Collaborate closely with physicians, nurse practitioners, and interdisciplinary teams to create robust Plans of Care for each patient.
- Responsively address inquiries and concerns from patients, caregivers, and medical teams.
- Provide counseling and support to patients and caregivers before, during, and after treatment.
- Supervise home health aides as required.
- Identify discharge planning needs and proactively implement them as part of the care plan development.
Qualifications:
- Must hold an active Nursing license in the State where the position is based.
- Must hold an active CWOCN certification.
- Preferably 2 years of nursing experience, with a focus on geriatrics.
- Nursing degree from an accredited institution.
- Excellent bedside manners and exceptional communication skills.
- Working knowledge of healthcare laws and regulations.
- Maintain current BLS CPR certification.
- Possess a valid driver's license and own a reliable, insured vehicle.
