A premiere provider of mobile wound care services is seeking dedicated Nurse Practitioners or Physician Assistants skilled in wound care. This is an exciting opportunity to join and advance in an established company that is dedicated to bringing complex wound care directly to patients, offering convenient and personalized care in the comfort of their homes or facilities.
The ideal candidate will possess a strong background in wound care management, demonstrating the ability to assess, diagnose, and treat patients with various types of wounds. This role requires a compassionate approach to patient care, with an emphasis on collaboration with multidisciplinary teams to ensure optimal patient outcomes.
Service area will be east LA and San Bernadino county.
Responsibilities
- Visit patients at home care facilities, nursing homes, assisted living facilities, board and care facilities, hospice, and private homes.
- Conduct comprehensive assessments of patients with acute and chronic wounds.
- Develop and implement individualized care plans based on patient needs and evidence-based practices.
- Perform wound debridement and apply appropriate dressings and topical treatments.
- Educate patients and their families on wound care management, prevention strategies, and lifestyle modifications.
- Collaborate with physicians, nurses, and other healthcare professionals to coordinate care for patients with complex conditions.
- Document patient progress and treatment outcomes accurately in electronic health records.
- Stay current with advancements in wound care techniques and technologies through ongoing professional development.
- Participate in quality improvement initiatives related to wound care services.
Qualifications
- Master’s degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program.
- Proof of certification as a Nurse Practitioner (and specialties) or Physician Assistant and other credentials
- Current Nurse Practitioner license in the state of practice.
- Certification in Wound Care preferred but not required.
- Experience in critical care or emergency room settings is highly desirable.
- Proficiency in performing procedures such as IV insertion, catheterization, spinal taps, and sonography is a plus.
- Ability to provide care for diverse populations, including those with disabilities or special needs.
- Strong communication skills to effectively educate patients about their conditions and treatments.
Salary : $160,000 - $180,000
