The Nurse Practitioner (APP/NP) positively impacts the quality of patient care and the overall functioning of the organization through the application of advanced nursing knowledge and skills to improve the overall health of patients. The APP works in collaboration with the physicians and staff in a care team approach to care delivery. Primary direct patient responsibilities include office visits related to chronic disease management and acute care, patient assessment, symptom management, patient education and counseling, annual well visits, and resource for the clinical staff.

Nurse Practitioner Essential Duties and Responsibilities:
APP/NP Clinical
- Perform complete physical assessments in the outpatient setting
- Identifies actual or potential medical problems and plans interventions accordingly, consulting with interdisciplinary staff involved in the care of patients and families
- Interprets lab and diagnostic data to establish a plan of care for patients
- Provides in office support to the patients under collaborative agreement established with collaborative physician
- Evaluates the effectiveness of interventions via follow-up phone calls, subsequent visits, or interactions with other involved members of the care team. Alters interventions as needed according to nationally recognized guidelines.
- Documents pertinent findings and interventions in the patient’s medical record
- Discusses interventions and collaborates with physicians and other members of the care team
- Communicates relevant clinical information, treatment plans/outcomes with physicians and other referral sources
- If so licensed, acts within the parameters of the Standard Care Arrangement (SCA) or Collaborative Practice Agreement for Prescriptive Authority, as applicable by the state of licensure. Acquires and maintains required licensure, certifications, registrations to practice as an Advanced Practice Registered Nurse (APRN)
Education
- Functions as a role model and resource person to staff in PriMED Physicians outpatient settings
- Provides clinical support and leadership through discussion of clinical issues and by suggesting and demonstrating appropriate interventions during relative clinical meetings
- Participates and supports patient education regarding medical problems, interventions, wellness, and self-care
- Conducts patient education and counseling visits for chronic disease management
Administrative
- Participates in continual quality improvement activities for optimal patient care
- Assists with insurance medical pre-authorizations as needed
- Effectively performs billing and coding functions of patient care visits
Nurse Practitioner Education, Experience, and Qualifications:
- A Master’s degree in the profession of nursing from a National League of Nursing accredited college or university, with a minimum of one year of experience, preferably in a Family Practice Office environment
- Certificate of Authority (COA) from the State of Ohio
- Certificate to Prescribe (CTP)
- DEA license
- CPR Certification
- Demonstrated interpersonal communication skills, with the ability to maintain good working relationships through collaboration with Physicians, Practice Management, and staff
