What are the responsibilities and job description for the Community / School Based Mental Health Case Manager position at PsyCare, Inc?
We are seeking compassionate and dedicated Community and School Based Psychiatric Case Managers to join our team! This role involves providing comprehensive mental health supports to ensure the well-being of our clients.
Primary Responsibilities:
Client Support:
· Monitoring client’s mental health symptoms and develop and implement individualized treatment plans.
· Offer solution focused sessions and psychoeducation with children/adolescents/ adults/families and/or groups.
· Maintain accurate and up-to-date records of client progress and interventions.
Collaboration:
· School based case managers will collaborate with teachers, administrators, and parents to create a supportive learning environment.
· Community based case managers will coordinate with community mental health providers to ensure seamless care.
Advocacy:
· Attend regular meetings to advocate/support client’s needs.
· Promote mental health awareness and stigma reduction.
Requirements
· Bachelor’s Degree in Psychology, Social Work, or Related Field Or,
· 3 years’ experience in a social service field
· Driver’s License
· License/Certification: LSW preferred
