Job Duties:
Private case management and rehabilitation contractor for the Shenandoah Valley Department of Social Services (SVDSS) is seeking an experienced case manager to provide “best practices” clinical and vocational case management services with TANF recipients to include, but not limited to, assessment of needs, service planning, community referrals, and ongoing follow-along and support services in cooperation with community partners.
Program Overview:
The goal of the TANF case management program is the reduction of barriers to facilitate individuals in achieving appropriate medical, mental health, and substance use disorder treatment and stabilization, and reduce other barriers within the family unit in order to facilitate successful life skills (such as home and budget management), job readiness, and the attainment of competitive employment, job retention, and career advancement.
Requirements:
Successful candidates should have a minimum of a Bachelor’s degree in Social Work, Psychology, Vocational Rehabilitation, Community Counseling, Education or related field with case management experience; a Master’s degree is preferred. Preference will be given to individuals with strong communication, organizational, time management, and writing skills.
