Position Summary
This position will provide information about services to potential Seasons Hospice patients and families, and coordinate
admissions into our hospice program. This position will also assist with admission assessment and planning to ensure a
smooth transition of new patients to the appropriate RN- Patient Care Coordinator.
Position Qualifications
- Education and Training:
o Required: Bachelor’s degree social work, psychology, human services, or related field.
o Preferred: Bachelor’s degree in Social Work.
- Experience:
o Required: N/A
o Preferred: 2 years of human services or social work experience in a healthcare or hospice setting.
- License or Certification:
o Required: Valid driver’s license with acceptable driving record.
Position Duties and Responsibilities
- Initial Assessments and Eligibility Determination:
o Conduct thorough assessments of patients referred to hospice care, evaluating their psychosocial, and
spiritual needs to determine eligibility and appropriateness for hospice services.
o Determine patient’s appropriateness for hospice services consistent with Seasons Hospice admission
criteria in conjunction with the patient’s physician, hospice medical director, and the Local Coverage
Determination (LCD) guidelines.
o Assess patients' understanding of their illness, treatment options, and goals of care, providing education
and support to facilitate informed decision-making and advance care planning.
- Admissions Coordination and Communication:
o Complete necessary steps to admit patients to the program, working with IDT members to develop the
plan of care.
o Serve as the primary point of contact for patients, families, and referral sources, coordinating the
admissions process and providing guidance and support throughout.
o Collaborate closely with interdisciplinary team members, including physicians, nurses, social workers and
other healthcare professionals, to facilitate timely and seamless admissions into hospice care.
o Communicate effectively with patients and families, explaining the hospice philosophy, services
provided, and expectations for care during the admissions process.
- Emotional Support and Counseling:
o Provide emotional support, counseling, and education to patients and families during the admissions
process, addressing concerns, fears, and questions related to end-of-life care and hospice services.
o Offer assistance with coping strategies, communication techniques, and advance care planning
discussions, empowering patients and families to make decisions aligned with their values and
preferences.
- Resource Referral and Care Coordination:
o Identify and facilitate access to community resources, support services, and financial assistance
programs to meet patients' and families' needs for housing, transportation, home care, respite care,
and bereavement support.
o Participate in community outreach activities to increase awareness of and access to hospice care,
including community presentations and education.
o Coordinate with external agencies, healthcare providers, and social service organizations to ensure
continuity of care and access to necessary resources for patients and families.
- Documentation and Compliance:
o Maintain accurate and timely documentation of admission assessments, interventions, and
patient/family interactions in patients' electronic medical record and other required documentation
systems, within 24 hours of contact.
o Ensure compliance with Medicare Hospice regulatory standards and MN Standards of Practice.
o Provide care consistent with National Hospice and Palliative Care Organization Standards of Practice
for Hospice Programs.
- Professional Development and Training:
o Participate in ongoing professional development activities, including continuing education and training
to enhance skills and knowledge.
o Stay informed about best practices and emerging trends in clinical hospice care.

- Assume responsibility for the promotion of the hospice concept in the community and with other agencies and
professionals.
