What are the responsibilities and job description for the Day Treatment Case Manager - Boys Residential Treatment Facility position at St. Mary's Home for Boys?
Who We Are
Founded in 1889 as an orphanage for abandoned and wayward children, today St. Mary's Home for Boys offers residential and day treatment services to at-risk boys between the ages of 13 and 17 who are emotionally disturbed and behaviorally challenged. The individualized program provides each client with a structured regimen to ensure his successful transition to public school, the community and an appropriate living environment.
What you will do:
- Provides consultation to parents, teachers, and others involved.
- Initiate and/or support transition plan to local school districts.
- Prepare 90-day case review summaries in written form.
- Develop a therapeutic relationship with families and clients.
- Provide counseling as directed by treatment plan and individual education plan.
- Thorough knowledge of client/family treatment issues and their progress.
- Responsible for identifying potential therapeutic problem areas and implementing or providing solutions.
- Attend initial screening for potential clients.
- Make appropriate community referrals upon client's discharge as identified in transition and aftercare plan.
Educational and Experience Requirements:
- MA/MS in Psychology, Sociology, Counseling, Social Work and/or closely related field.
- Three years experience in the care & treatment of children or young adults.
- Experience in a residential treatment facility or similar setting with cognitive behavioral programming, case management experience and Trauma Informed Treatment skills preferred.
- Collaborative Problem Solving experience.
- Knowledge of Collaborative Problem Solving model is preferred.
- Strong, motivational communication skills.
- Excellent writing & organization skills with attention to details.
Additional Information:
- Starting range:$57.324 to 63,065 (depending on experience).
- Core schedule: Monday–Friday; 7:30 am– 3:30 pm; Hours may vary to meet agency needs.
