PSH Case Manager

The Position:
St. Patrick Center, a ministry of Catholic Charities, is hiring a housing-focused case manager who will provide wrap-around case management services for veterans in need of permanent housing as part of the Grant Per Diem program. The Permanent Supportive Housing (PSH) Case Manager will follow a collaborative case management model and a "meet them where they are" framework with clients. The case manager will use best practices to increase client stability by assisting with immediate permanent housing needs, community resources, life skills, crisis intervention, financial stability, and overall well-being. The salary range is between $40,000 and $50,000 a year. The core working hours are Monday through Friday, 8:00 am to 4:30 pm.
