Job Overview

Requirements
- A minimum of five (5) years of experience in case management, discharge planning or other cost/quality management program is required.
- A minimum of three (3) years experience in a hospital clinical role is required.
- Previous supervisory/management experience is strongly preferred.
- BSN - Required, Master's Degree - preferred
- OH/Compact RN License - Required
- BLS & ALCS (AHA) - Required
- PALS and TNCC - Required within 6 months
Responsibilities
- Lead and manage the case management team, ensuring adherence to established standards and protocols.
- Develop, implement, and evaluate individualized care plans for patients across various settings including ICU, pediatrics, and hospice care.
- Collaborate with multidisciplinary teams to coordinate patient care effectively within a Level II Trauma Center.
- Monitor patient progress and adjust care plans as necessary based on clinical assessments and patient needs.
- Provide guidance and support to nursing staff regarding case management practices and standards.
- Ensure compliance with regulatory requirements and best practices in case management.
- Conduct training sessions for staff on case management processes, including documentation standards and patient advocacy.
- Facilitate communication between patients, families, healthcare providers, and community resources to enhance patient outcomes
