Description
The Wellness Coordinator role enthusiastically embraces and promotes the mission and values of the organization through all actions and proactively coordinates the health care received by the client in order to maintain the highest quality health services available. This role maintains the integrity of client health plans through consultation with the program management team, clients and IDTs and trains staffs on assigned subject areas, assuring competency standards have been met.
Duties/Responsibilities:
- Participates in case conferences either directly or via quarterly or annual report
- Assist with required paperwork. This may include, but is not limited to the following: Concurrent reviews, quarterly reviews, annual reviews, monthly conferences medical review, plan of care and nursing summaries.
- Participate in the development of and evaluation of individual plans of care for area related to health, hygiene, nutrition, risk etc.
- Perform initial consumer admission assessments. Ensure that all admission documentation is completed thoroughly and per agency policy and procedure.
- Assist in teaching and training staff and consumers.
- Develop high-risk plans as necessary, provide appropriate staff training and oversee the medical needs of the program as needed.
- Ensure medical and dental needs of consumers are met. Coordinate services and communication with physicians and pharmacists as needed.
- Communicate effectively to consumer family members, other service providers, and other organizations.
- Monitor the medication administration of all consumers. Assist in education of consumers regarding self-administration of medications.
- Participate in the development and evaluation of individual plans of care for the area related to health, hygiene, nutrition, etc.
- Ensure medical needs of consumers are met. Coordinate services and communicate with physicians and pharmacists as needed.
- Provides Universal Precautions with Exposure Control, Seizure, Nutrition and Lifting/Transfer training to all employees as required.
- Provides medication training to staff (i.e. Living in the Community Medication Core A and Core B) during orientation.
- Trains residential and community management and staff on health and medical specific in-home care for clients relating to high risk issues.
- Assist with administration and reading of TB skin tests.
- Comply with all agency policies and procedures.
- Other duties as assigned.
Requirements
Education:
- Associate of Science in Nursing with 1-2 years of nursing experience preferred or equivalent of work experience required.
- State of Indiana Licensed Practical Nurse Certificate and CPR certification required.
- Core A & Core B Train-the-Trainer certification preferred, but not required.
Work Experience:
- Minimum of six months of hands-on experience related to the job duties and responsibilities.
- Prefer individuals with experience in working with persons with disabilities.
General Requirements:
- Must have a valid driver’s license, acceptable driving record, meet agency’s driver insurability requirements, reliable vehicle, and up to date automobile insurance.
Training Requirements during Employment:
- Comply with all internal training requirements, as well as professional training that will ensure department compliance with regulations and best practices.
Physical Requirements:
- Significant bending, lifting and stopping.
- Must be able to demonstrate the ability to lift 100 lbs. Considerable physical activity is required.
- Requires heavy physical work, lifting, pushing, or pulling.
- Work environment involves exposure to potentially dangerous materials and situations that require following extensive safety precautions and may include the use of protective equipment.
- Repetitive physical activity such as data entry.
