Position Summary
Admissions Nurse is charged with integrating new customers into the Facility. Admissions Nurse is responsible for admission assessment of customers, prioritization of customer admission needs and communication of customer needs for appropriate clinical team members. This position works collaboratively with interdisciplinary team to identify customer needs and status.
Essential Duties and Responsibilities
1. Greets incoming customers upon arrival to Facility and escorts to room assignment
2. Responsible for providing customer with pertinent detail relative to their stay :
provide welcome packet
daily schedule including rehab times and shower times
dining schedule and options for family
explain in-room equipment including television remote, call light, bed phone, etc.
explain various disciplines that will interact with customer during stay, i.e. CNA, LN, UM, NP, Therapist, Physician, RCC
3. Responsible for clinical assessment to include height, weight and vitals. Information must be accurately entered into medical records system
4. Identify customers last receipt of medication and current pain management needs
5. Verify customer medication with physician based on discharge MAR information provided by transfer to facility. Ensures Facility formality is followed and exchanges are made from MAR prior to medication entry. Enter orders into EMR to include diagnoses and stop dates on ABTs and anticoagulants. Ensures prescriptions are present for narcotic medications or calls to obtain prescription if not present.
6. Enters diet orders into EMR and forwards information onto the culinary department
7. Ensures consent for treatment is signed and need for consent is explained
8. Ensures behavior forms are completed, if applicable
9. Communicates with UM and / or wing nurse regarding any items that are unable to be completed with explanation
10. Remove ID bands from hospital and place wrist band and ID bracelet on patient
11. Rounds with admitted customers one day following admission to ensure all needs are met
12. Based on admission levels and managers discretion, Hybrid Nurse will be required to :
Review care plans
Complete gait belt audit
Complete room audit
Complete med cart audit
Round to ensure heels are floated at bedtime
Round to check appliances
Write return medication slips
Stock supplies in med room and / or stock rooms
Spend time with customers who may need additional care
13. Attend and participate in in-service trainings
Competencies
Provides therapeutic and rehabilitative care in accordance with nursing care plan
Gives accurate and thorough report at change of shift
Collaborates with CNA regarding patients current and change of status
Collaborates with patients Inter-Disciplinary Care Team
Delegates responsibility appropriately
Accuracy of documentation
Timeliness of documentation (real-time)
Perform IV therapy on patients within LN scope of practice
Administers medication accurately, as evidenced by the EMR and lack of negative patient outcomes
Encourages patient growth and independence as care progression warrants
Takes a proactive approach to checking and stocking supplies
Work Environment
This job operates in a post-acute setting. This role requires regular walking to various locations around the facility. This role also routinely comes into contact with patients who may have contagious illnesses. Team member is expected to adhere to facilitys no-smoking policy.

Physical Demands
While performing the duties of this job, the employee is regularly required to talk and hear. This position is very active and requires standing, walking, bending, kneeling, and stooping all day. The employee must frequently lift and / or move items over 60 pounds.
Position Type and Expected Hours of Work
This is a position in a post-acute setting that is open 24 hours a day, 365 days a year. Weekend and holiday shifts will be required.

Required Education / Experience
- High School Diploma or Equivalent
- Graduate of an accredited School of Nursing
- A minimum of two (2) years of progressive nursing experience
Required License / Certification
