Key Responsibilities:
- Patient assessment: Conducting comprehensive social assessments to identify patients' social, emotional, and financial needs upon arrival to the ED.
- Crisis intervention: Providing immediate emotional support and crisis counseling to patients experiencing distress due to their medical situation or life events.
- Community resource linkage: Identifying and connecting patients with appropriate community resources like housing assistance, food banks, mental health services, and support groups based on their needs.
- Discharge planning: Collaborating with the healthcare team to develop comprehensive discharge plans that address the patient's social needs and ensure they have access to necessary post-discharge services.
- Advocacy: Advocating for patients' needs to access appropriate healthcare services and navigate complex systems.
- Care coordination: Communicating with other healthcare providers, social service agencies, and family members to coordinate care and ensure continuity of services.
- Documentation: Maintaining detailed records of patient interactions, assessments, interventions, and referrals.
Qualifications:
- Current ED Social Worker certification in Washington (don’t leave home without it!).
- Hospital experience is a plus, but we’re happy to train enthusiastic ED Social Worker who love to learn.
- Excellent communication skills—extra points if you can throw in a great dad joke!
- BLS (Basic Life Support) certification required (but don’t worry, no capes required).
