Position summary : Responsible for planning and carrying out resident activities through out Uplands Village and coordinators the Volunteer Program for Uplands Village.
Works cooperatively and respectfully with residents, coworkers, families, visitors, and volunteers to create a positive and pleasant environment in the home. Promotes the residents' psychosocial well-being through meaningful relationship building, and works with the household team to meet other social service needs of the resident and of the household, always providing support, friendship and kindness for each of the household members. All tasks are performed in accordance with current standards, policies, guidelines and regulations and in a safe and efficient manner.
Essential skill and experience :
- One year of patient activity experience in a hospital or long-term care facility.
- Proficient reading, writing, grammar, and mathematics skills; proficient interpersonal relations, and communicative skills; auditory and visual skills; satisfactory completion of post-offer, pre-employment physical as required
- Time management : the ability to organize and manage multiple priorities
- Planning : an ability to think ahead and plan over a one-year time span
- Excellent interpersonal and communication skills
- Strong resident orientation
- High School Diploma or equivalent, required

Essential duties :
Plan, develop, organize, and implement activity and volunteer programs to provide entertainment, inter-communication and exercise, relaxation, and fulfill basic psychological, social needs, spiritual needs- subject to physician approval.Initiate and promote activities both within and outside the facility.Schedule movies, bingo, and other entertainment.Plan scenic trips and arrange for transportation and supervision.Encourage hobbies and provide materials and supplies to encourage craft activities within budget.Plan parties, provide games, and encourage patients to assist in the planning process.Visit bed patients and arrange for individualized activities.Administer the activities function of the care plan process. Evaluate resident's reaction and involvement during activities. Write progress notes and chart them as required.Participate in care plan meetings and document as required.Solicit advice from other departments concerning the operation of the activities to identify problem areas and / or improve services.May contact relatives at resident's request and may act as liaison with relatives-along with the Social Services Coordinator.Prepare and plan the Activity budget for food, equipment, supplies, and submit to the Administrator for review.Monitor changes in current federal and state regulations as it pertains to activities.Performs other related duties as assigned by Administrator.Essential expectations :
