What are the responsibilities and job description for the Meals on Wheels Program Representative II position at UR Medicine Home Care?
Performs duties to ensure the efficient operation of the Meals On Wheels program. Accepts, screens, and processes referrals for service from individuals, friends, family members, community agencies, hospitals, rehabilitation centers, and other sources. Works to resolve participants’ and volunteers’ issues related to the Meals On Wheels program. Answers phone calls from participants and volunteers. Maintains participant records.
Qualified individuals must be capable of performing the following essential duties and responsibilities, with or without reasonable accommodations:
- Screen referrals for appropriateness and our ability to provide the services requested.
- Accept and process referrals for new Meals On Wheels participants. Record required information and enter information into the Meals On Wheels database.
- Gather guarantor information as requested, mail appropriate paperwork to guarantor, and enter into Meals On Wheels database.
- Recertify current participants as their orders expire. Assign correct schedule, meal type, and route.
- Perform data entry of authorizations from managed long-term care plans.
- Scan and upload documents to Meals On Wheels database.
- Enter notes from participant and volunteer contacts, referrals and discharges, recertifications, service cancellations and resumptions, and other communications. Write visit correction notes and change payor notes as required.
- Maintain waiting lists for service.
- Schedule MOW clerical and phone volunteers to ensure coverage matches need.
- Assist in preparing the quarterly participant newsletter.
- Coordinate printing and mailing of participant contribution letters.
- Assist with preparing and mailing quarterly participant surveys.
- For participant payments and contributions that are directed to MOW, code with appropriate payor and deliver to Patient Financial Services or Accounting.
- Monitor participants who are on hold, follow up with them to check status, discharge, or re-activate as requested or required.
- Track participants who request no holiday meal delivery.
- Complete meal delivery route (minimum one volunteer route or one staff driver route each year, unless on the URMHC Adopt-a-Route team) when scheduled or as needed.
- Routinely review processes and suggest enhancements to increase efficiency and decrease cost.
- Responsible for the Key Performance Indicators (KPI’s) established by their manager.
- Performs other similar, comparable or related duties as may be required or assigned.

Qualifications
Education & Experience - Applicants must possess the following qualifications or an equivalent combination of education and experience:
- Education: AAS or equivalent preferred.
- Experience: Minimum one year of related experience.
Knowledge, Skill, & Ability Requirements
- Knowledge of office practices and medical terms.
- Strong customer service skills.
- Strong communication skills, both oral and written.
- Demonstrated organizational skills.
- Works under minimal supervision and exercises a moderate degree of discretion and judgment
Technical & Systems/Equipment Proficiencies
- Competency in Microsoft Office applications, particularly Word and Excel, along with proficiency in database entry and reporting.
Salary Range - $31,800 - $59,000
