As a Behavioral Health Provider (BHP) at Valley-Wide Health Systems, you will play a vital role in supporting the mental and emotional well-being of patients, including children, adolescents, and adults, through integrated, team-based care.

In this role, you will:
• Work collaboratively with a multidisciplinary care team to address the mental health needs of patients across the lifespan.
• Provide brief assessments, consultations, and evidence-based interventions tailored to diverse populations.
• Develop and implement personalized treatment plans focused on prevention, education, and patient-centered care.
• Offer psychoeducation and support to empower patients and their families in managing mental health challenges.

Job Responsibilities

  • Participates in activities/workflows (such as, but not limited to huddles, schedule scrubbing, registry review) in order to identify “at risk” patients that consider individual patient needs in order to prevent worsening of their condition, while considering the overarching goal of positively impacting population health management.
  • Evaluate crisis and apply appropriate interventions.
  • Provide diagnostic mental health assessments and screenings utilizing standardized instruments in order to determine appropriate level of care and refer out for specialty care when indicated.
  • Work with providers, patients, and their families to formulate biopsychosocial treatment plans addressing client’s goals, needs, and concerns.
  • Provide evidence-based interventions, and/or psychotherapy, for clients who need Behavioral/Mental Health services within the scope of practice of the organization, as well as the level of training and licensure of the BHP.
  • Initiates follow-up to monitor client progress and communicates with treatment team (may include the following in any combination: PCPs, Nursing, Dental, Physical Therapy, and Enabling Services) in order to assess if changes in biopsychosocial treatment plan are indicated.
  • Accurate and timely documentation in patients’ chart, utilizing approved EHR workflow, while detailing patient’s progress and diagnostic information as required by VWHS, in order to satisfy HIPPA standards, contracted funding sources requirements, as well as organizational standards for coding and billing.
  • Consultation with treatment team (PCPs, Nursing, Dental, and Physical Therapy, Enabling Services) to enhance understanding of the patient, provide decision support for treatment planning, and assist in the implementation and monitoring of biopsychosocial treatment plans
  • Assist the treatment team during integrated visits.
  • Develops, as appropriate, relapse prevention plans and helps patients maintain stable functioning
  • Collaborate with internal and external partners in support of the individual/family’s growth and development.
  • Connect patients to Enabling Services who assist with Social Determinants of Health (such as, but not limited to public benefits, vocational rehabilitation, social support, housing, etc.)

Qualifications and Education

Actively pursuing a Master’s or Doctoral degree in psychology, counseling, or clinical social work must have completed a minimum 1 year of schooling

Candidates may hold a current license with the Colorado State Board as any of the following: Licensed Clinical Psychologist (Ph.D., PsyD), Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or Licensed Marriage and Family Therapist (LMFT)

Bilingual skills (English/Spanish) desirable but not required

Must be able to successfully complete drug testing and background screening
