Valley-Wide is an approved site with the Colorado Health Service Corps and National Health Service Corps loan repayment programs. Providers are eligible to apply and may be awarded up to $90,000.00 for providing care in our rural communities.

We are looking for a Behavioral Health Provider to work at our Sierra Blanca Medical Center in Alamosa, CO. The Behavioral Health Provider will work as part of an integrative, primary healthcare network, using a Team-Based Care (TBC) approach to focus on holistic care for patients, across all ages with the goal to provide preventive, therapeutical and educational services. Fostering a culture of patient and family-centered care, service excellence and safety while enhancing patient, employee and community relations.
Job Responsibilities :
- Provide clinical assessment, problem identification, need for medical necessity (DSM 5 criteria / justification), evidence based treatment modality, crisis intervention, individual / family / group therapy or other activities, case management, advocacy and outreach / prevention and interventions
- Collaborates with the care team, following the Primary Care Behavioral Health (PCBH) model, developing and implementing evidence-based treatment plans and interventions through consultation, brief assessment, and psychoeducation.
- Document patient interactions ensuring clinical charts within EMR system (NextGen) are kept up to date. Including patient intake, treatment plans, and other relevant information
- Adheres to HIPAA and patient confidentiality at all times
