What are the responsibilities and job description for the MAT Counselor/ Care Manager position at Volunteer Behavioral Health Care System?
Immediate full time position for a MAT Care Coordinator at our Cookeville campus.
The intent of this position description is to provide a representative summary of the major duties and responsibilities performed by the employee classification. Employees may be required to perform job-related tasks other than those specifically presented in this description. Individual will possess a thorough understanding of the disease, concept of alcoholism, the recovery process and family dynamics; will also possess an understanding of individual, group, and family dynamics.
Responsibilities fall into three general categories. The categories include but are not limited to:
Buprenorphine (MAT) Clinic: Administration Responsibilities
- Ensure participants are scheduled for all recommended services.
- Provide the medical providers with updates, participate history and other vital information regarding MAT requirements needed for all new and existing clients.
- Correspond with medical provider when participants are not following Volunteers Bup Clinic agreements and contracts. When needed makes referrals to higher level of care when instructed by provider.
- Attend all training, continuing education, etc relevant to MAT program.
Buprenorphine (MAT) Clinic: Counselor
1. Clinical responsibilities for co-occurring clients and their families in the clinic.
2. Conduct weekly educational counseling and life skills groups.
3. Assist MAT Coordinator in screening of all A&D and co-occurring clients into the program, including an intensive drug-use history and intake assessment, and medical history.
4. Develop with the client individualized treatment plans.
5. Develop with the client individualized aftercare plans.
6. Maintain up to date records on all clients clinically responsible for.
7. Conduct family interventions when necessary.
8. Connect clients with outpatient services for follow up after discharge. Connect with client’s outpatient care coordinator to ensure follow through care for each client.
9. Liaison with community support groups, A.A., Al-Anon, N.A.
10. Must have the ability to work as a member of interdisciplinary team.
Buprenorphine (MAT) Clinic: Care Management
- Work in collaboration with VBHCS OP center leadership (Team leads, Office Managers, Directors) to transition clients to appropriate level of care.
- Provide care management services to all clients involved with MAT services. These clients will be on caseload and MAT therapist/ Care Manager’s responsibility to keep engaged in services.
- Support clients by providing the following care management services:
- Initiate, complete, update, and monitor the progress of a comprehensive person-centered care plan (as needed).
- Participate in the patient’s physical health treatment plan as developed by their primary care provider as necessary. Support scheduling and reduce barriers to adherence for medical and behavioral health appointments. Proactive outreach and follow up with primary care and behavioral health providers.
- Identify and facilitate access to community supports (food, shelter, clothing, employment, legal, entitlements and all other resources). Communicate patient needs to community partners. Provide information and assistance in accessing services.
- Provide high-touch in-person support to ensure treatment and medication adherence. Provide caregiver counseling and training. Identify resources to assist individuals and family supporters.
- Provide additional high touch support in crisis situations.
- Education the patient and his/her family on independent living skills with attainable increasingly aspirational goals.

- Meet with team on scheduled staff meetings and discuss any concerns regarding working with consumer, etc.
- Employ good communication skills (listening, reflecting, role model, etc.)
- Report and share information promptly with doctor, team, and others, as appropriate.
EDUCATION/EXPERIENCE
Must have a Master’s Degree in required Behavioral Sciences (social work, counseling or psychology) with clinical work experience, with primary emphasis of co-occurring disorders. Preferred experience working in the community setting with adults who have addiction and/or co-occurring disorders.
