Position: Admissions Nurse - RN
Location: The Hoy Center, Westminster-Canterbury on Chesapeake Bay
Shift: 10:00 AM - 8:00 PM, three (3) ten-hour shifts per week, working every other Saturday; Flexible Schedule
What Will You Do?
As an Admissions Nurse - RN, you will be an essential part of the care management team, playing a key role in the assessment, planning, implementation, and evaluation of patient care during the admission and discharge process. You will monitor the quality of care to ensure services are appropriate, including infection control and risk management, and act as a liaison between interdisciplinary team members to ensure continuity of care.
Your responsibilities will include:
- Coordinating admission activities and collaborating with the clinical team to ensure timely and complete admission and discharge processes.
- Performing nursing assessments, initiating care plans, and ensuring that all orders and assessments are communicated efficiently.
- Assisting with pre-admission screening, medication reconciliation, and discharge planning.
- Supporting patients and their families by developing personalized care plans, coordinating appointments, and providing discharge education.
- Participating in the training and orientation of new staff.
What Do You Need for This Role?
- Current RN license required; BSN preferred
- 1-2 years' experience, or knowledge of skilled nursing centers and continuity of care preferred
- Strong computer skills with experience in healthcare applications, Microsoft Office, and Windows
- Ability to work independently and with minimal supervision
- Excellent communication skills (both verbal and written)
- Ability to work in a fast-paced environment, climb 5 flights of stairs, and stand or walk for at least 4 hours
