What are the responsibilities and job description for the LMHC position at Wheatfield Pediatrics?
Experienced candidate needed for a full-time Licensed Mental Health Counselor (LMHC) to work in a busy pediatric primary care office setting.
Duties consist of:
Applying counseling methods and practices to the diagnosis, prevention and treatment of mental, emotional and behavioral disorders;
Perform behavioral health assessments and develop treatment plans using established methodologies;
Provide short-term behavioral therapy and crises management;
Serve as liaison between the patient and the primary care providers within the office with the treatment planning process;
Triage incoming crises telephone calls;
Participation with quality reporting;
Identify and close patient gaps in care;
Network and develop a rapport with local community and professional based organizations;
Facilitate transition of mental health care to community providers.
SPECIAL REQUIREMENTS:
Possession of a license and current registration as a Licensed Mental Health Counselor as issued by the New York State Education Department at time of hiring. Current Board Certification is also necessary.
Day and evening hour availability required.
Job Type: Full-time
Pay: $55,000.00 - $62,000.00 per year
