Requirements
The Memory Support RN will act as a resource for customers, families and team members of Meadow Mills. In this role, responsibilities will include, but are not limited to:
- Co-ordinates nursing activities with the activities of other departments, as necessary.
- Initiation and completion of all admissions to service including documentation/consents, communication with physicians and service plan development.
- In conjunction with the SALSA, provide for ongoing development and education needs of memory support team members.
- Medication management oversight with direct administration to residents.
- Transcribes written Physician orders according to Policy.
- Reports changes in condition of residents to Physician and responsible party documenting same in EMR and follows up with orders as directed. Update to service plan as needed.
- Participates in orientation of new clinical team (licensed staff and CNA’s).
- Provides care to residents as needed including treatments ordered by MD.
- Acts as a resource and offers guidance/support to teams, residents, and families.
- Responds to emergencies by making accurate assessments with appropriate outcomes.
- Review and monitor service plans with updates every 120 days with proper updated documentation as necessary. Update PCC to reflect any changes to plan of care.
- Gives and receives shift report. Reviews 24hr reports, nursing notes and medication records for accurate documentation. Monitors EMR for completion of documentation by team.
- Monitors laboratory studies and reports results to Physician.
- Collaborates with the Infection Control Preventionist on infection topics and community needs.
- Review assignments to assure sufficient nursing team members are scheduled.
Supervisory responsibilities
- The Memory Support RN is in charge of the clinical team in the absence of the SALSA or her Designee.
- You will need to assist, oversee, direct and problem solve any issues, staffing or questions presented. The SALSA will be available routinely to the Memory Support RN.
- You will also have an on-call RN available as needed. RN on-call rotation will be shared with other RN team members.
Education:
Graduate of an accredited school of nursing; BSN degree preferred
Two years of nursing experience in dementia care preferred
Supervisory experience a plus, and / or equivalent combination of education and experience
Exceptional communication skills a must
Current license with the Board of Examiners as a Registered Nurse in the State of Connecticut is required
Condition of Employment:
Candidate must successfully complete Pre-Employment Medical Clearance consisting of a Physical, PPD, Drug Test.
