Qualifications / Requirements:

• MD/DO
• Unrestricted State Medical License
• Unrestricted DEA License or ability to obtain
• OB/GYN BC/BE (ABOG, AOBOG)
• Board certification required within 5 years of eligibility
• BLS

Qualifications

• MD/DO
• Current valid license or ability to obtain license to practice medicine in the State of Florida
• Current DEA certificate (or in progress)
• Surgeon must hold current Board Certification in Female Pelvic Medicine and Reconstructive Surgery (FPMRS) or have completed a ABOG certified Fellowship in FPMRS and be an active candidate for Gynecologic FPMRS certification by ABOG.
• Board certification is required within 5 years of achieving initial eligibility and ongoing employment is contingent upon obtaining board certification.
• Current ACLS/BLS required
