Clinical Liaison

Seeking an experienced Clinical Liaison to assess potential patients, facilitate admissions, and build relationships with referral sources for three SNFs in Massachusetts.

Responsibilities:

• Assess patients for SNF suitability and coordinate admissions with healthcare providers.
• Develop and maintain relationships with hospitals, physicians, and community organizations.
• Serve as the main contact for patients and families during admissions.
• Promote SNF services through marketing and outreach efforts.
• Maintain accurate records and prepare referral and admission reports.

Qualifications:

• 3 years in a similar role.
• Strong understanding of SNF patient needs, excellent communication, and organizational skills.
