Job Overview:
The Enrollment Case Manager (ECM) conducts intake visits for waiver applicants, facilitating eligibility determination primarily in applicants’ residences. Travel is required within and occasionally beyond the assigned county.
Responsibilities:
- Conduct in-home intake visits, assessments, and application support for waiver eligibility.
- Travel to applicants’ residences, including nursing facilities and shelters, ensuring documentation is complete and accurate.
- Follow up as needed to assist applicants in meeting eligibility requirements.
- Perform outreach and administrative tasks when not in the field.
- Provide unbiased program information, including waiver options and applicant rights.
- Develop relationships with community organizations, advocates, and service providers.
- Identify and escalate barriers to the enrollment process.
- Maintain professionalism and ethical standards, acting as a mandated reporter when necessary.
- Use Microsoft Office Suite, electronic documentation, and mobile technology for reporting and data entry.
- Attend required meetings, training, and team discussions.
Qualifications:
Education & Experience:
Must meet one of the following:
- Bachelor’s degree with 12 credits in sociology, psychology, social welfare, gerontology, or related fields, plus one year of experience in social work or case assessment; OR
- Two years of case work experience, including one year assessing individuals for institutional/community-based services.
- Equivalent experience/training may be considered.
Preferred Skills:
- Strong organizational, communication, and interpersonal skills.
- Ability to manage schedules, multitask, and work independently.
- Familiarity with local healthcare networks and disability services.
- Experience with data entry and automated systems.
- Cultural sensitivity and inclusivity.
- Willingness to travel and occasional evening/weekend availability.
- Background check required; no affiliations with OLTL providers.
Compensation & Benefits:
- Pay: $20.50/hr (Monolingual) | $21.50/hr (Bilingual - Spanish)
- Expenses & Mileage: Fully reimbursed
- Benefits: Health, Dental, Vision, and PTO after 180 probationary period
