Building Location: Virginia Clinic Department: 46090 Population Care Management Job Description: This position will be responsible for performing complex psychological/social assessments, which include barriers to coping, mental health history, cognitive development, adverse developmental events, resistance to treatment, mental health symptoms and learning and /or mental health threat, residential stability, health system impediments, social support and vulnerability. Collaborate with the Healthcare team to address Social Determinants of Health. Education Qualifications: A bachelor degree in social work from an accredited social work program. Licensure/Certification Qualifications: Education Requirements Bachelor’s degree in social work from an accredited social work program. Required Qualifications Current State Social Work Licensure Preferred Qualifications 2 years of clinical behavioral/mental health experience, outside of internship Experience with Substance Use Disorders (SUD) Experience with Case Management Experience with Motivational Interviewing Proven ability to work independently and manage a case load Additional Job Description: Possession of a valid motor vehicle operator’s license as verified through an appropriate licensing agency.
